Our Owners
James Middleton Jr.
MD, PhD
Founding owner of FMC on July 11, 1977 after graduating from the University of Louisville School of Medicine and trained in family practice at Trover Clinic in Madisonville. He holds a PhD degree from the University of Kentucky and is a member of the American Academy of Family Physicians as well as being certified through the American Board of Family Medicine.
Evelyn Salisbury
MD
Coming to Hart County as the first pediatrician in 1980 having graduated from the University of Louisville School of Medicine and is a member of the American Acedemy of Pediatrics. She trained in pediatrics at Norton Children's Hospital in Lousiville, Kentucky and specialized in allergy care services.
Ann Marie Hemmer
MD
Graduate of University of Kentucky School of Medicine joining FMC in 1999 after seeing an increasing demand of another pediatrician and later becoming an owner of the practice. She trained in pediatrics at the Medical Center of Central Georgia. She is a member of the American College of Pediatrician and the Children's Health Center Task Force.

APRN Staff
Continuing to care for the people of all ages of Hart and surrounding counties our team of nurse practitioners are excited to serve those that come through our doors. Each have multiple years of experience treating acute and chronic health issues and strive to provide the highest quality of care delivered in a confidential, safe and caring environment to patients across the age spectrum. Our nurse practitioners have extended experience such as pulmonary, critical care medicine and sleep medicine also education that makes them valuable to us and our community.

Our goal is to provide professional medical service of the highest quality for patients while maintaining an enviornment that fosters the professional and personal growtih of the individual staff member.
Family Medical Center of Hart County has grown over the years because it has recognized and met the needs of the community and the patients. Responding to the needs of the patient is still the highest priority of Family Medical Center of Hart County and we expect this dedication will mean continued expansion of our services in the future.
