Fees and Payment
We accept most major insurance plans. Bring a copy of your insurance card with you to all appointments. Copayments will be collected during the check-in process and are required at the time of service.
Statements are generated and mailed once a month. Payment is due promptly. If you have questions about billing or payment options, please call the office and speak to someone in our Business Office.
Insurance Providers
Please review the provider listing provided to you by your insurance carrier or employer, or contact them for verification. FMC will file your claim with your insurance carrier, however, should your insurance plan not pay your claim within 60 days from the original date of claim submission; the charges become the responsibility of the patient. Our insurance department will be glad to assist you with problem claims.
Motor Vehicle Accident (MVA)
You will be asked to sign a Motor Vehicle Accident release form. This authorization allows us to communicate with your insurance carrier about your treatment. Kentucky State law requires us to bill auto insurance of the vehicle you were in, regardless of fault.
Workers Compensation
When seeing a provider for a work-related illness or injury, inform the receptionist when you register that your visit is work related. Doing so will enable us to obtain the necessary information to file a correct claim.
Your employer will be contacted to confirm they are aware of the illness/injury because they have certain reporting requirements and they must verify financial responsibility. You will be asked to sign a Worker's Compensation release form. This authorization allows us to communicate with your employer and their insurance carrier about your treatment.
Financial Responsibility
The financial responsibility for service rendered rest with the patient or his/her guarantor, regardless of insurance coverage. Any financial benefits you may receive from your insurance company are strictly a matter of settlement between you and that insurance company. Insurance companies require us to collect any co-payment at time of service.
Living Will
A Living Will gives you a notice in decisions about your medical care when you are unconscious or too ill to communcate.
